Become part of an innovative trans-disciplinary care team! As a Certified Home Health Aide, you will provide personal care services to patients in our Skilled Home Health line of service in the Sonoma County area. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.  Essential Duties & Responsibilities Include (but are not limited to):
  • Assists home care patients with personal care, transfers in and out of bed, ambulation, and with use of the bedpan
  • Plans, shops and prepares nutritionally sound meals and/or special diets as ordered by the physician
  • Assists with medications that are ordinarily self-administered
  • Assists with prescribed exercises which patients and home health aides have been taught by appropriate health personnel
  • Reports changes in the patient's condition and needs
  • Documents care of patient on appropriate forms
Education/Experience:
  • High school diploma or general education degree (GED); or related experience and/or training; or equivalent combination of education and experience.
  • Completion of an approved Home Health Aide training program.
Certificates, Licenses, Registrations:
  • Current CPR certificate
  • Current certification to practice as a Home Health Aide in the State of California.
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

  • Communicate with the Intake and Admissions department regarding hospice referral/admission.
  • Assist Director of Provider and Community Relations in negotiating or redefining contract provisions as necessary and in accordance with relevant laws and regulations.
Education/Experience:
  • AA or B.S. degree in Nursing from an accredited Nursing School.
  • Minimum of one experience in a home health/ hospice setting.
Certificates, Licenses, Registrations:

Their primary responsibilities are psychosocial assessment, crisis intervention, consultation, education, complex discharge planning, brief therapeutic interventions, community linkages and supportive services for identified patients, families and staff. The Inpatient Hospice Social Worker provides supportive services to help meet patient/family social, emotional and financial needs related to the impact of a terminal illness, death, loss and grief, in compliance with organization policies and procedures and applicable laws and regulations.   • Assess special needs related to cultural diversity including communication, role of family members and special traditions.
  • Develops materials and provides education to patients, families, staff and community related to psychosocial needs and interventions.
  • Establish treatment goals and revise counseling interventions as indicated. Evaluate patient/family response to psychosocial interventions; identify obstacles to compliance and assist in understanding goals of intervention.
  • Assist patients, family and staff in utilizing community resources. Address patient/family questions and issues. Evaluate patient/family response to intervention(s) when referred to community agency and satisfaction of the service(s) provided.
  • Provide information and preparation of advance directives, funeral planning, and the transfer of responsibilities regarding health care and related decisions.
Education/Experience:
  • Master degree in Social Work from a school accredited by the Council on Social Work Education or Masters Degree in mental health discipline.
  • Minimum one-year’s social work experience in a health care setting.

Certificates, Licenses, Registrations:

  • Current Licensed Clinical Social Work (L.C.S.W.) certification in the State of California may be required for some positions.

This Social Worker will be responsible for providing supportive services to help meet patient/family social, emotional and financial needs related to the impact of a terminal illness, loss and grief, in compliance with organization policies and procedures and applicable laws and regulations. This position is required to work 2 weekend shifts a month. Essential Duties & Responsibilities include the following:
  • Assess patient/family social and emotional factors in order to determine capacity and potential to cope with life threatening illness and death.
  • Provide individual and family counseling and support services to patients/families including crisis intervention. Identify family dynamics and communication patterns.
  • Establish goals and revise counseling interventions as indicated. Evaluate patient/family response to psychosocial interventions; identify obstacles to compliance and assist in understanding goals of intervention.
  • Identify developmental level of patient/family and obstacles to learning or ability to participate in decision-making.
  • Assess special needs related to cultural diversity including communication, space, role of family members and special traditions.
  • Assist patients/families in utilizing community resources. Address patient/family questions and issues. Evaluate patient/family response to interventions when referred to community agencies and level of satisfaction with the services provided.
  • Assess for potential risk for suicide and/or abuse or neglect.
  • Assess for potential risk for medication misuse and diversion
  • Assess patient/family ability to meet financial obligations and offer appropriate resources as indicated.
  • Provide information and preparation of advance directives, end of life planning including the transfer of responsibilities regarding fiscal, legal and health care decisions.
Education/Experience:
  • Master degree in Social Work from a school accredited by the Council on Social Work Education.
  • Minimum three years supervised social work experience in community mental health/public health or related field. Hospice, Home Health or Hospital experience preferred.

By the Bay Health is currently seeking a registered nurse to serve as a Nurse Case Manager in Sonoma County. This person will be responsible for the provision of skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families. The ideal candidate will have finely tuned assessment skills and be knowledgeable about Hospice care best practices. Essential Duties & Responsibilities include (but are not limited to):
  • Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals and interventions.
  • Coordinate all patient/family services and prioritization of needs with the members of the transdisciplinary team.
  • Use a case management approach and refer to other services as needed.
  • Coordinate the implementation of the plan of care and insure care coordination and case management for each patient.
  • As applicable, apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient’s eligibility.
Education/Experience:
  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing.
  • Experience in acute care/ home health and/or hospice setting preferred.
Certificates, Licenses, Registrations:
  • Current license as a registered professional nurse in the State of California.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
