We are pioneers in hospice care at home, only the second such agency in the country, and the first in California.
Feel free to contact any of our By the Bay Health physicians any time during a patient’s illness. We are happy to consult with you on pain management, hydration, and other aspects of end-of-life care, as well as help you determine if it is time to refer to another service line.
We urge you to refer to hospice care early. There is no penalty for referring too soon. Our medical team can help determine if it is time. Should your patient come on our service, you can stay involved with care while we treat symptoms and provide spiritual, emotional, and practical support to patients
and their families.
We offer two programs for patients requiring palliative care—a consultative option and a comprehensive option. The choice is largely determined by coverage options and location.
SKILLED HOME HEALTH CARE
We have combined the expertise of two formerly independent agencies, Incare and Healing at Home, to offer patients recovering from injury or surgery, or managing chronic illness the option to receive treatment in the comfort of their own home.
We have one of the very few pediatric hospice and palliative care programs for children in Northern California.
We provide hospice care, consultative palliative care, and grief support for patients in Skilled Nursing Facilities and Residential Care Facilities.
We also offer training for facility staff members in best practices of end-of-life care.
