By the Bay Health is currently seeking an RN or LVN to serve as a Night On Call (NOC) Nurse in our Marin location. The ideal candidate will have finely tuned assessment skills and be knowledgeable about Hospice care best practices  in order to triage incoming calls and attend to patients in the field as needed. This nurse will be covering 4-5 weekday, midnight -8:30 am shifts. Essential Duties & Responsibilities include (but are not limited to):
  • Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
  • Under the guidance of the Resource Manager, responsible for patients/families including assessing, planning, implementing and evaluating nursing services consistent with the plan of care.
  • Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family.
  • Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issue.
Education/Experience:
  • One year experience as a professional nurse in the past three years or; Bachelor’s Degree in Nursing from an accredited School of Nursing.
  • Experience in acute care/ home health and/or hospice setting preferred.
Certificates, Licenses, Registrations:
  • Current license as a registered professional nurse in the State of California.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.
  • Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

We are  currently hiring an Admissions Nurse for our Hospice line of service to work in the San  Mateo County area. This RN will provide information about services to potential agency patients and admits patients to agency's total care category of service in coordination with Team Leader(s). This position is a full-time, swing shift from 12 pm to 8 pm.  Essential Duties & Responsibilities include (but are not limited to):
  • Screen and process inquiries for admission to By the Bay Health's total care category.
  • Determine patient's eligibility for hospice care.
  • Make telephone contact and/or assessment visits with potential hospice patients to provide information on agency services and/or assess patient's status.
  • Assist with implementation and coordination of services required for discharge of patients from hospitals into agency's program of care.
  • Maintain current documentation in computerized clinical documentation system according to agency policies and procedures and applicable laws and regulations.
  • Assume initial responsibility for assessing patient/family needs and consulting with attending physician and other hospice staff members.

  • Facilitate transition of patient/family to Patient Care Team in a timely manner.
  • Manage patient/family care plan and accompanying services between time of admission and transition to Nurse Case Manager as necessary.
Education/Experience:
  • One year experience as a professional nurse in the past three years or; Bachelor’s degree in Nursing from an accredited School of Nursing.

By the Bay Health is currently seeking Bereavement Associate / Interns (post grad) and Bereavement Trainees for our Grief Counseling training program in our Marin, Sonoma and San Francisco locations. We are recruiting now for Winter and Spring 2021. Interns and Trainees provide individual, family, group and school based Grief Counseling to bereaved Hospice families and members of the community. Interns/Trainees receive:
  • Weekly individual and group supervision meeting requirements of the BBS and Graduate Degree Programs.
  • A wide range of applicable educational in-services and learning opportunities.
  • An hourly stipend of $22/hr.*
  • Extensive training in clinical documentation in an electronic medical record.
  • Development of interns and trainees theoretical orientation in a supportive environment.
  • Professional counseling experience in a fast-paced medical agency
Prospective candidates must:
  • Hold a Master’s degree or be currently enrolled in an accredited Master’s Degree Program in Counseling Psychology or Social Work. Appropriate registration with the BBS required.
  • 1+ years of experience conducting on-going counseling preferred.
  • Be knowledgeable about Loss and Grief Counseling techniques
  • Be able to commit to 20-24 hrs./week, including one evening per week, for 12 months & travel to satellite sites, as needed.
  • Have availability which includes Mon. or Fri. & the 1st Wednesday of each month.
  • Be proficient in basic Microsoft Office applications. Experience with documentation in an electronic medical record preferred.
